Information Management/Medical Records
You may request copies of your medical records and/or x-ray films for the Health Information Management Department after your discharge. Only complete records can be released unless needed by other health care providers for continuing care. We will charge a reasonable fee for copies according to Florida Law. Federal law requires that we give you information on our Privacy Practice Notice. This law, referred to as HIPAA or Health Insurance Portability and Accountability Act of 1996, became effective April 14, 2003. Santa Rosa Medical Center’s Notice of Privacy Practices will be presented and explained to you upon registration to our facility. If you have further questions or concerns about our Privacy Policy or would like a copy, please contact our Privacy Officer at (850) 626-5160.
