Take a Weight Loss Surgery Assessment
You may be a candidate for a weight loss procedure if you are:
    - 
    Between the ages of 18 and 65 (If you're younger or older, your case may be considered)
- Someone with a BMI 30 – 35 on a case by case basis
- Someone with a BMI > or equal to 35 with major obesity related co-morbidity(s)
- Someone with a BMI > or equal to 40
- 
    No more than 100 lbs over ideal body weight
- 
    Struggling with an obesity-related condition like hypertension, heart disease or sleep apnea
- Currently trying or have tried to lose weight through other means such as diet, exercise and behavioral changes with only short term success
- 
    Psychologically ready for weight loss surgery and willing to commit to a whole new lifestyle
- 
    Not drug or alcohol dependent or have at least one year of sobriety
